The patient was diagnosed as COPD associated with right heart failure and bronchiactasis. They explained the symptamology in chronological order which is helpful for diagnosing the patient. As she works in paddy field, her symptoms may be due to paddy dust or insects or some bacteria .they have clearly explained the primary etiology of her disease . the and also about the mechanism . They have done proper examinations and investigation required for this case with a very good presentation and the treatment for all her symptoms are given properly.

Case-2:
The patient was diagnosed with liver abscess with pain in abdomen and loss of appetite since 1week and fever since 2 days. They have diagnosed the case accordingly by taking detailed history of patient which helps to treat him in a better way. They also did the general and systemic examination and also radiological examination which helps to find out exact location of abscess and the spread of infection. They did proper investigations necessary for the case which help in effective treatment. Their presentation was good showing the orderly info and images of patient and with investigation reports.

The patient is presented with an abdominal pain and vomitings since 5 yrs and is a chronic alcoholic and he got treated temporarily in the past yrs .because of the continuation of alcohol intake his symptoms got worsen and he complained of constipation and burning micturition and throbbing pain radiating to back.
As the symptoms are severe for the early recovery they gave the antibiotics in the injectable form and also stated about his nutrition because as he is chronic alcoholic which lead to thiamine deficiency. As his pancreas are effected , to decrease the exocrine secretion of pancreas they have given octreotide inj. Their line of treatment has kept in the presentation in a good way

A patient with the chief complaints of fever since 6 days and shortness of breath since 2 days and also with dry cough and dimnition of sense of smell and she was diagnosed with COVID Pneumonia. They diagnosed the case based on the history and the symptamology, they also mentioned in the case about drug history of patient which is really helpful for her treatment line. They did the necessary investigations including the HRCT Scan for the progression of the disease. They presented the case with adequate information required for the diagnosis and explained clearly with pictures and flowcharts.

The patient was presented with shortness of breath, chest pain and B/L Pedal edema. The patient is a chronic smoker and alcoholic but assymptomatic since 1 month and suddenly developed SOB-Grade3.They did 2Decho and ECG and found out some abnormalities. As the patient had no improvement in symptoms he was advised for higher investigations. The video of the patient reveals the complete picturisation to analyse the disease. I feel the presentation was very accurate and complete information is analysed.

Case-6:
The patient was diagnosed with pancreatitis, pseudocyst and left bronchopleural fistula. They have clearly explained his symptamology in a sequence and did the necessary investigations required in this case. Clinical examination is shown clearly in the video. They clearly stated the indications and mechanism of action. The efficacy of the drugs was mentioned and standard sources were used. The presentation was very clear and comprehensive

Case-7:
The patient presented with chief complaints of fever since 4 days and pus in urine. After careful examination of the patient he was diagnosed with hydrouretronephrosis. For the proper understanding of a case the important things are highlighted and they have done the necessary investigations with pictures in their presentation. KUB X ray was also done showing the patient bilaterally. They also collected the urine samples and did the examination.it is a very good presentation with pictures and the line of treatment was written clearly.

The patient is presented with pain abdomen and vomiting and was diagnosed as Acute on Chronic pancreatitis. They did the ultrasound abdomen and identified the cause . their presentation was very brief and they did not explained about the mechanism of action. If they have included the pictures of the ultrasound and the patient it would be clear.

The patient presented with chief complaints of slurring speech and deviation of mouth that lasted for 1 day. The patients history was taken in detailed and was diagnosed with Cerebellar ataxia to acute CVA with infarct in the right inferior cerebellar hemisphere. Systemic and motor examinations are shown properly in the video. CT Scan revealed the cerebellar infarct and2d echo showed diastolic dysfunction. They have included all the pictures of patient and the investigation reports and diagnosed it clearly. In overall it is a very good presentation

And she is the known case of hypothyroidism since 5 yrs so she was under thyronorm 100 mg OD
She was diagnosed with Atrial fibrillation after detailed investigations and her biochemical report showing severe hypothyroidism possibly relating to her refractory atrial fibrillation. Systemic and abdominal examination was done thoroughly.ECG,2D Echo complete blood picture is done for identifying the exact etiology of the disease. The case was very well explained and the medication prescribed was effective. The investigations were presented in a proper format with respective subheadings and pictures were included.
They did not stated about the reason why the patient developed hypothyroidism for 5 yrs.

Renal :
This is a case of a 45 yr old male with acute kidney injury on chronic kidney disease with uraemic encephalopathy
The patient with chief complaints of
-altered sensorium
-history of fever since 10 days
- pedal edema with anasarca
- shortness of breath present even at rest
He is the known case of hypertension and chronic kidney disease since 5 yrs and is on conservative treatment. Serum creatinine levels are estimated day by day to find any variations in treatment. Systemic and CNS examinations are done to identify the etiology. ECG, CBP, Ultra are investigated and patient is kept on treatment for few days. The treatment plan was well described. The overall presentation was comprehendable.

We the students of 3rd semester just began our clinical postings in online mode. Due to pandemic it became difficult in terms of study through online than the offline. This has become a difficult challenge in terms of syllabus, coping up with patients and communication gap. But with the help of the professors, PGs and Interns our task has become easier even though it is an online mode. They really make us understand the scenario of each and every aspect .We are really thankful to to them who are working hard to make us learn in a n effective manner. We will try to utilise this opportunity to maximum and do our best to learn.
